How to requesting for money or benefit according to your policy ?

When claiming for indemnity or requesting for money or benefits as stated in the policy, one need to contact the insurance company as quick as possible. There are different reasons why one would make a claim as listing below...

Death caused by illness and diseases - the policyholder would need to inform the insurance company within 14 days with proof, including insurance policy, receipt of the last payment, death certificate of the insured, house registration of the beneficiary, and national identity card of the beneficiary.

Death caused by suicide - the policyholder would need to prepare the same proofs, as stated in the policy, as when one dies as a result of an illness or disease. In addition to that, there are two more documents needed to be prepared which are a copy of daily report of police officer and autopsy report.

Death caused by accidents - the policyholder would need to prepare proofs as same as the case when it is death caused by illness or diseases. Aside from that, the policyholder also needs a copy of daily report of police officer, a copy of daily report of police officer after returning from the scene the accident occurred, and autopsy report.

However, if the policyholder did not pass away from causes mentioned above, but was affected and became disabled, lose limbs, or want a reimbursement for medical fee, the policyholder would need to inform the insurance company within 10 days and prepare some documents which are a compensation claim form, a receipt of medical fee stated for the first until the last day of treatment in hospital, and others such as X-ray film.

On the other hand, if one reached the end of the agreement, one would only need to prepare the insurance policy and national identity card before contacting the insurance company.
